GSSI Out-of-State Appointment Travel Checklist

Traveling out of state for a gastroparesis appointment can feel like a lot to keep track of. This checklist from GSSI is here to help you pack and prepare, one step at a time. You’ve got this!

Before You Go

  • Photo ID and insurance card
  • Appointment details and clinic address
  • Referral, medical records and recent test results
  • Current medication list and medicines in original packaging

Pack for the Trip

  • Phone, charger and emergency contacts
  • Comfort items and a change of clothes
  • Foods, drinks or feeding supplies that work for you
  • Questions and notes for your care team

Follow your care team’s guidance for medicines, meals and feeding supplies.

If you are traveling to Louisville for care, GSSI may be able to help with non-medical needs like hotel stays, food/grocery cards, and local rides when funding is available. Learn more on our Request Assistance page: https://gpsupportservices.com/request-assistance/ — and see our hotel guides for travelers at https://gpsupportservices.com/hotels-near-the-uofl-gi-motility-clinic/ and https://gpsupportservices.com/hotels-near-vanguard-medical/

General information only — not medical advice, and not a promise that GSSI will pay for any one person’s stay. Help depends on funding available.
